Post Pinched Nerve vs Prostatitis
Hope you all had a great Thanksgiving.

Sometimes I wonder if I have a pinched nerve somewhere in the back INSTEAD of Prostatitis. Would love to hear some opinions to see if the symptoms line up with others.

1. Pain during ejaculation. Can radiate pain down side of leg to my foot and big toe. Is a sharp pain.
2. Feels like a I need to urinate MORE when I am sitting or laying down.
3. Random pains in groin/stomach area.

Am on Doxy now (3rd day). Dealing with this off and one for a year. Per usual, urologist found nothing (even with renal/prostate/bladder ultrasound. I can pee fine.

I do have back pain and I wonder if - instead of prostatitis - it could be a nerve issue. Just wondering if anyone ever had the back checked for possibile different diagnosis. Getting a correct diagnosis totally sucks.

It very well can be nerve pain! I have similar stuff going on. I have a doxy script just sitting in the cabinet but I refuse antibiotics now as they can really screw your stomach up. Your best bet is to get to a Chiropractor or Physical therapist to figure out whats going on or an interventional pain management doctor. (The ones in your area who do nerve blocks, nerve condition studies, ESI injections and ect) If you are like me, medications, even hydrocodone won't take the pain away but usually these type of Doctors can figure out what's going on with your back and send you to the proper specialist if needed. (Such as a Neurologist)

Thanks for the reply! Same here with Doxy. Been in my cabinet since April but figured what the hell I will give it a shot. Toughest thing is figuring out if it is actually coming from something other than the prostate.

I have been chatting with a neuro and may visit. An MRI can't hurt and maybe some blood work i guess.

This stupid disease seems to be a process of elimination.

If you have back pain and pain running down your leg that sounds like a very high chance of a nerve issue. Your antibiotics are probably helping at this point because they also are known to suppress neurinflammation as a side benefit.

Yes, if muscles are pinching the nerves then they're going to pinch even harder when they're all clenching together from an orgasm.

As a counterpoint, I have pain and numbness running down my legs as well when prostatitis pain is at a 3/5 or above. Mine is definitely not a nerve issue though (it's an infection). Of course it could be just a nerve issue in your case but it's best to explore all possibilities.

Good to know. I totally read your entire last post lol. Hope you are doing better.

Such a weird thing. I did the deed 4 hours ago. Hurt when it happened. Pain was the usual thighs to feet and of course in the penis. And even big toe. Bad for a half hour and starts to get a bit better but not go away completely.

Ill then wake up on day and itll he gone for weeks to months but comes back. And like you i cant find a pattern with it.

When this first happened i do recall hurting my back the day before. But still, cant find a pattern.

Prostate454, I read through your thread and it sounds like your latest tests have turned up negative for infection. Based on the number of abx you've taken I would really consider you're fighting some combo of neuroinflammation, nerve pinching, and/or central sensitization, especially with your concomitant tinnitus (two . Are you familiar with the Wise Anderson Protocol?

I'm not sure how far you are but you mentioned you are coming to Philly to see Dr. Pontari. Based on my visit he will most likely be pretty shocked at the number of abx's you've been on and want to do an eps urine sample not semen. If you are traveling to Philly I would actually recommend Dr Whitmore (though it wouldn't hurt to see both). She gave me some more non-traditional options that I think are used more in the IC world but may help including Urogesic blue which is a combination nsaid, bacteriostatic, and some kind of anti-spasmodic for the urinary tract and valium suppositories and some other bladder supplements recs (these overlapped with prostatitis supplements which makes me think these two conditions are more closely related than we think) and have you go on an IC diet. Even though she works more with women I found that she seemed more knowledgeable and gave me more new information to work with than any uro I've seen.

Thanks for your reply. My last test was only several days off antibiotics and they screwed up the test order by requesting an anaerobic culture with an aerobic transport so I have to be a little suspicious. If this Monday's test comes back clear then I'll feel pretty confident that there is no longer a significant infection. My pain doesn't feel like nerve impingement to me, it's never sharp and I've never had any sort of injury down there and maintain a very healthy and athletic lifestyle. And of course according to my last CT scan, my prostate enlarged by about 1cm in length and width between the period of 11/2015 to 9/2016, which was when the symptoms increased the most. Prostate is also pushing up against my bladder and seminal vesicles because it is enlarged. If I can solve that problem then I can solve my pain. There could be some neuroinflammation from having irritated nerves for so long, I def see how that could be a thing. The tinnitus originally started while on a 6 day course of methylprednisolone which has known ototoxic side effects and now drugs that can be ototoxic such as gabapentin and vancomycin make it worse.

This summer when I was going to internal PT I bought Headache in the Pelvis - my copy is sitting in my bedside table. I also do the internal PT stretches myself loosely based on the protocol. It helps a little but not enough to resign myself to years of sticking my finger up my rectum.

Thanks very much for your report on Pontari. I don't have any appointment with him yet, and your experience does make me a bit sad, but it's good to know. It's about a 4hr drive each way for me so I'd prefer not to waste more of my time if not necessary so the intel is appreciated. I'll do some research on Dr. Whitmore if I do end up heading that way, it can't hurt to try and kill two birds with one stone.

Sorry, didn't mean to derail the OP. It does sort of sound like it could be nerve related in his case, especially if the exams show nothing (mine DREs always show enlarged prostate). Maybe some pelvic PT is in order to test out the theory? It certainly sounds like the sort of case that the wise protocol might be beneficial for.

No worries at all! It's crazy that something so small can cause so much pain when inflammed. One day, a cure has to be found.

I went to a neurologist yesterday and, according to her, I have a herniated disc at L4. I guess that kind of makes sense with my symptoms. Gonna try PT for 6 weeks. I firgure it will either make it better or really make it worse. Either way, the process of elimanation game shall progress. Sometime I think I am just insane lol.

I hope you feel better soon. I've heard good things and bad things about the Headache in the Pelvis book. Read some samples but haven't pulled the trigger just yet.

consider going to a musculoskeletal expert and asking about osteitis pubis.. im have similar pains to yours, 1 year on i just had a diagnosis..
